APTPMA Membership Requirements
1. Aptpma membership form filled in completely and signed by the representative of the unit. (prescribed form available from aptpma office)
2. Membership fee (Please Confirm From APTPMA Office)
3. Aptpma annual subscription (Please Confirm From APTPMA Office)
4. Copy of C.N.I.C of the representative.
5. (4) photographs of the representative.
6. Copy of sales tax registration certificate.
7. Copy of N.T.N certificate.
8. List of machinery completely.
9. Copies of the utility bills (electricity, gas, wasa) (last three months).
10. Copy of sales tax return submitted last month.
11. Copy of income tax return (Recently Submitted)
